About LARIS Rehabilitation
LARIS Rehabilitation is a full service rehabilitation vendor providing comprehensive vocational services to persons with disabilities on behalf of employers, insurance carriers and attorneys. Vocational assessment and counseling services are also available to non-disabled individuals. Services are often utilized in workers’ compensation cases, liability proceedings, and divorce cases or other situations requiring an assessment of an individual’s vocational or educational potential, career related interests and vocational options. LARIS Rehabilitation is located at Silvernail Village in Pewaukee, located in Waukesha County, WI. Services are provided throughout Wisconsin. A satellite office location is in Manitowoc, WI.
Referral Material Guidelines
General guidelines for selecting referral materials to send in advance of the independent vocational evaluation (IVE).
- Initial report of accident such as the:
- Motor Vehicle Accident Report
- Work Comp First Report of Injury
- Medical records concerning treatment related to the accident
- Permanency reports and/or any medical info (such as a functional capacity evaluation outlining work or activity limitations.)
- Educational records
- Employment/personnel records
- Income/earning records such as a detailed FICA earnings report from Social Security, tax records, or employer’s wage reports
- IME reports
- Other vocational reports and or vocational rehabilitation records
- Other case specific information
